Adjunct Faculty Members
Required Tax & Employment Paperwork

If you are a new adjunct instructor or did not teach for us during the previous academic year, you are required to complete and submit the following forms (last updated August 24, 2009): 

Click the links above to download the forms. The forms listed above are also available in the School for Adult Learning office, Esch Hall 103M (or in the case of the tax forms, in the Payroll Office, Esch Hall 155).

New instructors must meet with the Contract Coordinator to set up a payroll account, to create a faculty account in the Registrar's system, to create an HR account with Human Resources, and to take care of all required tax and employment paperwork. You will be provided with all necessary forms when you meet with the Contract Coordinator. During your visit, he will also explain how to obtain your faculty ID card, your campus parking permit, and your university e-mail account—and take care of all other support matters on your behalf.

Please make an appointment to meet with the Contract Coordinator as soon as you have been offered a position by a Dean or Department Chairperson. Bring all of the completed forms listed above when you come to campus for your appointment.
